At the time of registration Parent(s) or Legal Guardian(s) MUST provide the following documents:

**Proofs of Residency (two required)

Current mortgage statement, property tax bill, closing papers, or lease agreement.
AND
Current utility bill, bank statement, voter ID, or payroll check.
- The proofs must be an official statement with the parent/guardian's name and address.

**Valid Driver's License

**Birth Certificate

**Dental Screening (Kindergarten and Young 5's only) - see Oral Health Assessment Form here

**Court-ordered legal guardianship documents, if applicable

**Special Education documents, if applicable
